In late 1971 or early 1972 an OH-6 Loach practicing autorotations at Ft. Benning (Lawson Army Airfield) autorotated on top of another helicopter (another Loach, I think) killing the two instructors and the two students. The IP of the upper OH-6 was a 1st Lieutenant, Putnam I think, who was a TAC officer in the Infantry OCS course. I don't see that incident recorded.
Also, in 1972 or 1973 a AH-1 Cobra impacted in the range area of Ft. Campbell, KY during gunnery practice killing both crewmen. The accident was attributed to target fixation, with the pilot bearing down on the target and "placing his aircraft in an attitude, altitude, and airspeed from which recovery to controlled flight was not possible prior to ground impact." I don;t see that incident recorded either.
