Since it is now released to the public.
The crew chief died due to injuries received after the wheel chocks became wedged on the collective in a dive, and the pilot could not recover. The crew was participating with other acft in an assault demo for VIP's. In the court martial, the pilot was found guilty of "aggressive" flying, at the request of the passengers.

Correct...the acft did not develop a mechanical problem. The CE did not stow the chocks.
